Coaching Agreement

I (Client) am committed to a creating a coaching alliance with Roz Applebaum (Coach). I commit to creating a successful alliance that supports me in reaching my goals and living the life I want.   I agree to the following business arrangement: The services provided by the coach are delivered virtually using the Zoom platform. All sessions during the course of this agreement are provided at $150 per hour. Or you can opt to purchase a package of 4 sessions for $500 payable up front. Payment is expected prior to each session. Coaching, which is not advice, therapy or counseling; may include action plans and homework assignments. The client is the sole decision-maker in the coaching process. Any, and all actions or consequences resulting from the coaching sessions are the responsibility of the client. The client releases the coach of all liability pertaining to the services rendered in the coaching relationship.
